This is the part that changes what you should do about it, so it’s worth getting right.

The Loop Nobody Mentions

1. Switch to alternate-day dosing
This is the highest-value change and the most counterintuitive, so it deserves the detail.
Conventional advice is to take iron daily, often in divided doses. That turns out to be wrong on absorption grounds. Taking iron raises hepcidin, a hormone that blocks further iron uptake, and hepcidin stays elevated for around 24 hours. So the second dose of the day, and the next day’s dose, arrive while the gate is still partly shut.
In a randomised trial, iron-depleted women given 60mg of labelled iron on consecutive days for 14 days absorbed a geometric mean 16.3% of it, while those given the same doses on alternate days for 28 days absorbed 21.8% (p=0.0013). Total absorbed iron was also higher on the alternate-day schedule — 175.3mg versus 131.0mg. The reasoning is mechanistic — fewer contact events with less unabsorbed iron should mean less irritation — and it’s a sound inference, but it hasn’t been directly tested for that endpoint. Even so, it’s an unusually good option: you plausibly reduce side effects and improve absorption, which almost never happens. Raise it with whoever prescribed your iron.
2. Change the form of iron
Not all iron salts behave the same in the gut.
- Ferrous sulfate — cheapest, most prescribed, and the one with the side-effect evidence above. Usually where people start and often where the problem starts too.
- Ferrous gluconate — lower elemental iron per tablet, so gentler largely by virtue of a smaller dose.
- Ferrous fumarate — higher elemental iron content; not obviously gentler.

3. Lower the dose
Standard doses are often higher than necessary. Because hepcidin caps absorption anyway, a large dose doesn’t deliver proportionally more iron — it mostly delivers more unabsorbed iron to irritate your gut. Many people do just as well on a lower dose taken less often. This is a prescriber conversation, not a self-adjustment, but it’s a very reasonable one to open.
4. Take it with food

6. Consider intravenous iron
If oral iron is genuinely intolerable, or you’re not absorbing it, IV iron bypasses the gut completely. Modern formulations are given as one or two infusions and have a good safety record. It’s a specialist decision and not a first step, but it exists — and the meta-analysis above found substantially fewer gut side effects with IV than with oral iron, which is precisely the point.
The Vitamin C Trap
Almost every iron leaflet tells you to take it with vitamin C or orange juice. For someone with reflux, that advice deserves scrutiny.
The rationale is sound in principle: ascorbic acid keeps iron in the absorbable ferrous form. But orange juice sits at around pH 3.5 and vitamin C tablets are acidic by definition, so you’re deliberately adding acid to a gut you’re trying to protect — and if you have LPR, dropping throat pH below about 4 is exactly what reactivates pepsin. The benefit is also less certain than the advice implies. Getting Iron From Food
Not a replacement for treating diagnosed deficiency, but it reduces how much supplementation you need.
Haem iron from animal sources is far better absorbed than non-haem iron from plants. Black Stools: When It’s Normal and When It Isn’t
This one matters, and it’s the most useful safety point in the article.
